Indiana Dental Association Relief Fund is located at 550 West North Street, Suite 300, Indianapolis, IN 46202. Indiana Dental Association Relief Fund is a 501(c)(3) non-profit benevolent organization dedicated to supporting members of the dental community across Indiana during times of personal crisis, severe hardship, or sudden tragedy. Headquartered in Indianapolis, the fund serves as a compassionate safety net for dentists, dental team members, and their families. Led by Executive Director Douglas Bush, the Relief Fund reflects the core values of professional solidarity and philanthropic care. By providing financial grants to those experiencing debilitating illness, accidents, natural disasters, or unexpected loss, the fund helps colleagues maintain financial stability and dignity through difficult circumstances.

Our services are intended for adults who can form a binding contract. We may request information to confirm your eligibility and reserve the right to approve, decline, or discontinue service in accordance with these Terms and applicable law.

Certain features require an account. You agree to provide accurate, current information and to keep it up to date. You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. We offer two-factor authentication (2FA) to help protect your account; you are responsible for keeping your contact information current so you can receive verification codes. Notify us promptly of any unauthorized use of your account.

You agree to use our services lawfully and respectfully, to comply with applicable laws, and to treat our staff and other users with respect. You agree not to harass others, misrepresent your identity, or engage in unlawful conduct in connection with the Site or our services.

Where fees apply, the applicable amounts and payment terms are presented at the time of enrollment or registration. Unless otherwise stated at the time of purchase or required by law, fees are non-refundable. Any refund requests will be considered in accordance with the policy stated for the applicable service.

When you register for a service or event, you agree to any additional rules or requirements stated for that activity. We may modify, reschedule, or cancel services or events, and will make reasonable efforts to notify affected users.

By creating an account or contacting us, you agree that we may communicate with you by email regarding your account and the services you register for. If you opt in to text messaging, you may receive confidential relief-application status updates and account verification codes (2FA / one-time passcodes) from Indiana Dental Association Relief Fund; message frequency varies and message and data rates may apply. You can cancel text messages at any time by replying STOP, or reply HELP for assistance. Your mobile number and SMS opt-in consent will never be sold, rented, or shared with third parties or affiliates for marketing purposes. Full details are in our SMS Terms & Conditions. You can manage your communication preferences at any time, and you may unsubscribe from non-essential messages.

You agree to use the Site only for lawful purposes.
